Each HMSA member (subscriber and dependents) will receive one membership card. Members should carry a card in their purse or wallet. Members can keep a photocopy of their card at home as a spare or take a photo of the card with their smartphone.
HMO members will have their health center and primary care provider listed on the cards.
Membership cards are mailed to the HMSA plan subscriber (one card for the subscriber and for each dependent) when the subscriber is newly enrolled, when a different plan is elected, when duplicate cards are requested, or when HMO members change their primary care provider or health center.
Present your HMSA membership card to your health care providers. The information on the card helps your provider determine what benefits are available to you and where to submit claims.
