Plan certificates are documents (similar to your medical and dental plan’s Guide to Benefits) that contain benefit information for your rider or add-on plans to your medical and dental coverage. Rider plans that may have a plan certificate include plans for services such as drug, vision, and chiropractic services.
The information in your certificate supersedes information contained on this site.
You may have access to view and print your plan certificates online through your HMSA My Account.(If your plan certificates aren’t available online, please contact your employment office or HMSA Customer Relations to request a copy.)
To find your plan certificates through My Account:
- Register and log on to My Account.
- Click on the Benefits tab on the top left of the page or View Benefits on the right side of the page.
- On the Benefits page, click on the PDF icons listed in the chart with Guide to Benefits and Summary of Changes.
