• How to Get Copies of the Drug Claim Form

  You can print the Prescription Reimbursement Claim Form from My Account on hmsa.com. You can also request copies by mail or in person.

• How Many HMSA Cards Will I Receive?

  Each member, including dependents, will get two membership cards in the mail. If you lose both cards, you can print a temporary one through My Account.

• HIPAA Certificate of Creditable Coverage

  We no longer generate Health Insurance Portability and Accountability Act (HIPAA) certificates of creditable coverage.
