You may request duplicate HMSA membership cards online. Just log on to My Account, hover over Profile, and click Request membership card.
You’ll receive a duplicate card within 10 business days.
You’ll receive the same number of cards previously provided to you, unless:
- You canceled or added a dependent.
- A dependent turned 18.
You can also use My Account to print a copy of your HMSA membership card.
Even if you don’t have your HMSA card with you, you can still receive services from any of your HMSA participating providers. Your providers have online access to your subscriber number and membership information.
However, if you urgently need a card, or if you don’t have your subscriber ID number and need it to register for My Account, call HMSA Customer Relations.
