One of the unique features of your Dual Care plan is the health risk assessment, a questionnaire that you’ll need to fill out. The HRA is designed to help you:

  • Be aware of your health and well-being issues.
  • Identify areas you may need to improve.
  • Set and work on goals.

Complete your HRA and send it back to us through the mail or by email or fax.

With HMSA Akamai Advantage® Dual Care, you’ll be assigned to a health coordinator or community health worker who’ll review your responses to the HRA, work with you to assess your health needs, and develop a care plan especially for you.

Your health coordinator or community health workers will work with you to make sure you’re following your care plan and staying on track with your goals. They may call you occasionally throughout the year for follow-up care. You can contact them if you need help or have questions about your health and well-being.

FAQs

Do I need to complete an HRA?
Yes. The Centers for Medicare & Medicaid Services requires you to complete the HRA for you to be in HMSA Akamai Advantage Dual Care.

Why do I need to complete the HRA?
The HRA is an important part of managing health conditions you may have. It’s also a good opportunity to prevent serious health problems. You’ll receive help and support from a health expert along the way.

When do I need to complete the HRA?
You need to complete the HRA within 90 days of enrolling in HMSA Akamai Advantage Dual Care and then once every year. You may have to complete it more frequently if you have certain health conditions.
