FAQs

What services does my plan cover?

Depending on your plan, HMSA health plan benefits may include counseling services, puberty suppression therapy, continuous hormone replacement therapy, gender affirming pre-surgery services, and gender affirming surgeries. To confirm whether a specific service is covered under your plan, refer to your Guide to Benefits in My Account, or call us using the phone number listed on the back of your HMSA card, or complete this form to request a callback.

Who’s eligible for these services?

Eligibility depends on several factors, such as age, diagnosis, medical and mental health conditions, and other clinical requirements. Eligibility criteria vary by service. To better understand what may be required for specific services, talk with your health care provider or contact HMSA Customer Relations for general guidance.

How can I get approval for gender-affirming services?

After you and your health care provider determine which services are appropriate for you, your provider can submit a request for approval, if required, based on your plan benefits.

How do I know if my doctor or facility is an HMSA participating provider?

The easiest way is to ask your doctor or the facility providing your care. You can also visit hmsa.com and use Find a Doctor. Providers listed in our database are HMSA participating providers.


Department of Health Sexual and Gender Minority Services Directory

In March 2022, the Hawai’i SGM Workgroup released the third edition of the Hawai’i SGM Services Directory. This is an opt-in directory of organizations, individuals, and health care professionals that offer identity-affirming services for sexual and gender minorities.
