One of the unique features of your Dual Care plan is the Health Assessment, a questionnaire that you’ll need to fill out. The Health Assessment is designed to help you:

  • Be aware of your health and well-being issues.
  • Identify areas you may need to improve.
  • Set and work on goals.

Complete your Health Assessment and send it back to us through the mail or by email.

With HMSA Akamai Advantage® Dual Care, you’ll be assigned to a health coordinator or community health worker who’ll review your responses to the Health Assessment, work with you to assess your health needs, and develop a care plan especially for you.

Your health coordinator or community health workers will work with you to make sure you’re following your care plan and staying on track with your goals. They may call you occasionally throughout the year for follow-up care. You can contact them if you need help or have questions about your health and well-being.

Do I need to complete a Health Assessment?
The Centers for Medicare & Medicaid Services requires a health assessment. HMSA encourages you to complete the Health Assessment so we can better understand your health needs and connect you with the right support.

Why do I need to complete the Health Assessment?
The Health Assessment is an important part of managing health conditions you may have. It’s also a good opportunity to prevent serious health problems. You’ll receive help and support from a health expert along the way.

When do I need to complete the Health Assessment?
You need to complete the Health Assessment within 90 days of enrolling in HMSA Akamai Advantage Dual Care and then once every year. You may have to complete it more frequently if you have certain health conditions.
