When a behavioral health practitioner's office is located in an area where it is likely the provider will have a high volume of patients, an on-site evaluation of the office is performed during initial Provider Credentialing. If site deficiencies are reported, the practitioner will be asked to correct the deficiencies and the site will be reevaluated to ensure the safety and well being of HMSA members.

To determine the high-volume areas, HMSA identified areas by zip code where its data showed that the highest number of patients had been treated. When a behavioral healthcare provider opens an office in one of the five zip code areas listed below, a site evaluation will be performed.
