HMSA accepts the following as adequate documentation of submitting DAW 1 (Brand medically necessary, do not substitute) or DAW 2 (Patient requested brand) codes:
- Written documentation on applicable hard-copy prescriptions
- Electronic documentation for DAW1 or DAW 2 codes that is maintained within the member’s pharmacy profile
