HMSA's claims processing system is programmed to validate that the diagnosis codes listed on the UB-04 form match the services rendered. ICD-9-CM codes generally will fall between 140-199 (as well as V58.1) and ICD-10-CM codes generally will fall between C00 – C96 (as well as Z51.11 and Z51.12) for chemotherapy-related diagnoses.
