HMSA’s Plan for QUEST Integration Members covers the prostate specific antigen (PSA) test as a screening for men age 50 or older and for men who exhibit any of the following:
- A history of malignant neoplasm in the prostate region
- Symptoms of carcinoma, neoplasms or hyperplasia of the prostate
- Unspecified urinary obstruction
