Original Effective Date:

Current Effective Date:

Physical examinations normally are covered for children as EPSDT screenings. However, other routine examinations outside the EPSDT periodicity schedule, such as sports physicals and pre-placement physicals, also are covered. Most physical examinations for adults also are covered and include examinations to determine health status and pre-employment physicals.

Physical examinations that are not covered include but are not limited to:

  • Physical exams for employment when the patient is self-employed, or as a requirement for continuing employment, (i.e., truck and taxi drivers’ licensing, other P.E.s) as a requirement for continual employment by the state or federal government, or by private practice
  • Physical exams and immunizations for travel purposes (foreign or domestic)
  • Physical exams and/or psychological evaluations as a requirement for Hawaii or other state drivers’ licenses, or for the purpose of securing life insurance policies and other plans
  • Physical exams to determine disability exemptions for immigration and naturalization purposes.
