QUEST Integration – Locally Assigned Codes

Locally assigned codes are not HIPAA-compliant and generally must not be used on claims submitted to HMSA’s Plan for QUEST Integration Members. The only exception applies to administrative procedure codes (Z9014 and Z9005) on paper claims when used to identify primary insurance payment information. All other miscellaneous services that are specific to QUEST Integration must be billed with HIPAA-compliant codes. A list of these services and applicable HIPAA-compliant code is found later in this section.
