In vitro fertilization and artificial insemination are not covered. Members wishing to undergo such procedures must be advised that QUEST Integration does not cover them and that if she wishes to proceed, she will be responsible for the full cost of the procedure and any other related services. The member must sign a statement attesting that she requested the non-covered service and will pay for all costs.
