QUEST Integration – Hospital Admission Notices

Admission notices must be sent when a patient is already in the facility and is transferring from a lower level of care to an acute level due to a change in the medical condition. Notices may be faxed to 808-944-5611 on Oahu.

Admission notices must include:

  • Patient’s name
  • HMSA QUEST Integration member ID number
  • Date of admission
  • Diagnosis
  • Medical record number
  • Hospital name

Failure to report admissions timely will result in payment reduction.

Routine newborn admissions should be faxed to report the birth of a newborn to a QUEST Integration member. If a newborn will be confined beyond the normal newborn stay due to a change in status to a surgical or medical case, an admission notice must be submitted to ensure full payment for the entire stay. Admissions for QUEST Integration-Net members do not need to be reported.

HMSA QUEST Integration will notify the PCP of their patient’s hospital admissions only if the member is in service coordination. All other information will be located in Coreo™. Notices are to be maintained in patient’s medical record.
