• Surgical dressings, splints and casts
• Equipment directly related to provision of the procedure
• Materials for anesthesia
• Administrative, recordkeeping and housekeeping costs
• Laboratory, X-ray or diagnostic procedures directly related to the surgical procedure

Services Not Included in ASC Rate

• Physician services
• Laboratory, X-ray or diagnostic procedures (other than those directly related to the surgical procedure)
• Prosthetic devices
• Ambulance services
• Leg, arm and back braces
• Artificial limbs
• Durable medical equipment for use in the patient’s home
