EUTF Retiree Benefits and Claims Information

How to identify which members have EUTF Retiree plan

  • Login to HHIN+ and look up the member’s benefits.
  • Member has one of the following coverage codes:
    • 449 – HSTA VB retirees WITHOUT Medicare. This became effective 1/1/2019. Prior to that, they had coverage code 668.
    • 563 – EUTF retirees WITH Medicare coverage.
    • 667 – EUTF retirees WITHOUT Medicare coverage.
    • 668 – HSTA VB retirees WITH Medicare coverage (as of 1/1/2019).

EUTF Retiree nutritional counseling benefits

  • Services are covered only when all of the following are met:
    • Must be ordered by a valid referring physician under an individual treatment plan.
    • Services are necessary to optimize a treatment program or mitigate the effects of an illness or condition.
    • Services must be received from a registered dietitian.
  • Bill with code S9470, not CPT 97802. Precertification is not required.

EUTF retirees and chiropractic benefits

  • HSTA VB members (coverage codes 449 and 668) have chiropractic benefits through American Specialty Health chiropractic rider coverage code C19.
    • C19 only covers chiropractic visits, not DME/supports/appliances.
  • Other EUTF retiree plans (563 and 667) are grandfathered plans and do not cover chiropractic services.

EUTF Retirees and dependent coverage information

  • Retirees can cover dependents that are full time students ages 19-24 for medical, drug, vision and dental.
