Erythropoietin

When billing for the erythropoietin drug (revenue code 634 or 635), enter the number of 1,000-unit doses administered in form locator 46. The dosage for erythropoietin must always be billed in units of 1,000.

Outpatient facilities and dialysis centers also must enter the total amount of erythropoietin administered in form locator 39 using value code 68.

Note:Use revenue code 635 when the dosage is 10,000 or more units (if number in form locator 46 is 10 or more). Use revenue code 634 when the dosage is less than 10,000 units.
