Agent Authorization

To authorize a billing agent or staff member to sign claim forms for your practice, complete the Agent Authorization [PDF] form and mail or fax it to HMSA as instructed on the form.

 You can:

• Print a copy of the blank form, or
• Type in the information on the PDF form and print a completed copy.
