Eligibility

Am I eligible?

You're eligible to enroll in the plan if you:

  • Have a permanent mailing address in Hawaii and
  • Aren’t covered by Medicare Part A, Part B or SSDI and
  • Are taking a minimum number of credits:
    • Fall and Spring semesters
      • UH and Chaminade undergraduate students need six or more credits.
      • UH graduate students need four or more credits.
      • Chaminade graduate students need three or more credits.
    • Summer semester
      • All students need three or more credits.

    1 credit of thesis (700) or dissertation (800) research also meets the credit requirement.

What if I don’t make the credit requirement?

You’re still eligible to enroll in the plan if you:

  • Provide your class schedule with a letter from the appropriate department that confirms the credits you’re taking are all you need to graduate.
  • You were enrolled in the HMSA Student Plan during the previous Spring and are enrolling in Fall but aren’t taking classes during the summer.

After enrolling and paying your bill, you can email your exception request to studentplan@hmsa.com.

Will I need anything to prove my eligibility?

You may be asked for:

  • Your class schedule.
  • Your UH or Chaminade University student ID number.
  • Other information that will help confirm your eligibility.

A signed HIPAA authorization form is required for HMSA to work with your school to determine your eligibility.

Plans

  • You can purchase coverage for yourself or you can add your spouse and children.
  • Repatriation coverage is required for international students and students with a J-1 visa.
  • Enrollments for students are accepted only during the open enrollment dates for each semester. Once the deadline has passed, you won’t be able to enroll until the next semester.

What are the benefits?

Each plan comes with medical and drug coverage. You can add dental benefits, repatriation, and life and accidental death and dismemberment benefits if you want.

Medical

Covers services received from doctors and hospitals.

  • $50 annual deductible.
  • $2,500 annual copayment maximum for single coverage, $7,500 copayment maximum for family coverage.
  • $0 annual exam, preventive screenings, and immunizations.
  • 25% coinsurance* for services like urgent care, emergency room, doctor or specialist visits, radiology, lab tests, hospital room and board, surgery, etc.

*Services that apply to your plan’s deductible or the amount you pay for covered services before your health plan pays.

Drug

Covers prescribed medications.

  • $10 for generic drugs.
  • $20 for preferred brand drugs.
  • $20 plus $45 cost share for non-preferred drugs.
  • $100-$200 for specialty drugs.

Repatriation, life, and accidental death and dismemberment

Covers life insurance and transportation of one’s remains to their home country.

  • Up to $25,000 for repatriation (required for international students).
    • Repatriation benefit applies only to students insured under a plan option that includes life and accidental death and dismemberment (AD&D) insurance and dependents of insured students if the dependent is a J-2 visa holder.
  • Up to $30,000 for life and accidental death and dismemberment.
    • Life and AD&D benefits applies only to students insured under the plan option that includes life and AD&D insurance.

Dental

Covers services received from dentists or dental centers.

  • $0 dental exams, cleanings, topical fluoride, and preventive care X-rays (excluding periapical).
  • 30% coinsurance for fillings, root canal, gum maintenance, and basic care X-rays (periapical).
  • 50% coinsurance for crowns, bridges, and implants.
  • The waiting period is 12 months for bridges, dentures, implants, and crowns.

Students who enroll should save a copy for their records.

If you're an international student attending UH, you'll need to submit this page to the UH Student Health Insurance Office for proof of enrollment.
