Condition Care Program

HMSA has a Condition Care Program to support you with your health and well-being goals to maintain or reach your optimal health.

We work closely with your doctors to help coordinate your care, including your plan of care, medication, and other support services. We want to make sure that you have access to the care and support you need to manage your health and well-being.

This program is voluntary and available at no cost to HMSA members.

How will I work with my care manager?

You have access to a team of Hawaii based care managers, which includes registered nurses, certified diabetes care and education specialists, registered dietitians, and health coaches. Care managers connect with you or your caregiver by phone, virtual visit, or in person. You can meet or talk with them from your home or in a provider’s office, at an HMSA Center, or another location whenever you need support for your lifestyle and health goals.

You’ll receive:
  • Support and information on how to access health screenings, tests, and treatments.
  • An action plan that you can use if symptoms get worse.
  • Tips on healthy eating and nutrition.
  • Help to manage stress.
  • Tips for increasing physical activity.
  • Help to set and achieve health goals.
  • Guidance to address needs such as transportation, finances, living conditions, and cultural and language needs.
  • Resources to support your health and well-being.
  • Help with understanding your medication.

Manage your Diabetes

Want help managing type 2 diabetes? Sign up for free HMSA Diabetes Self-management and Education classes. Classes are every second and third Wednesday of the month, 1-3:30 p.m., online or in person at the HMSA Center @ Honolulu. Free validated parking in the HMSA parking garage on Sheridan Street.
