Disclosure Statement

In order to be an HMSA participating provider, all providers must pass:

  • HMSA’s initial credentialing process: Providers must submit their information to HMSA.
  • HMSA’s re-credentialing process: Providers are asked to review and update any information every three years.

Providers are expected to let HMSA know of any changes as soon as possible.

Providers
HMSA doesn’t use quality measures, member experience measures, cost-related measures, or any other criteria to select practitioners for participation in our plans.

Hospitals
HMSA doesn’t use quality measures, member experience measures, patient safety measures, cost-related measures, or any other criteria to select hospitals for participation in our plans.

There may be constraints when providers and hospitals supply their own data to HMSA. It is important you know what is confirmed, and what is not.

Physicians


Name and Gender

Limitation: The provider gives us the info.

Specialty

The provider's area of practice, such as branch of medicine, procedures, age range of patients, body systems, or types of diseases.

Limitations: None. HMSA confirms this.

Hospital Affiliations and Privileges

The provider includes the hospitals he or she is approved to provide patient care and may include admitting privilege.

Health Centers

A group of physicians from which all health care services are provided or arranged.

Board Certified

Provider’s area of training or area in which they have passed a medical specialty exam.

Limitations: None. HMSA confirms this.

Education / Training

A provider’s initial training (i.e., medical school and internship) and any further training (e.g., residency and fellowship).

Accepting New Patients

The provider lets us know if he or she is able to take on new patients.

Term Date

If a termination date appears under a provider’s location, that location is either closing or leaving the HMSA network as of the indicated date. Please call the provider’s office or HMSA if you have additional questions.

Provider Languages

The provider lets us know if he or she speaks a language other than English.

Accreditation

A hospital is certified by an accredited board, such as The Joint Commission and the Commission on Accreditation of Rehabilitation Facilities, to meet the board’s quality standards.
