Patient Panel Report
This report is the total list of your patients for the current month, with name and contact information. Each month, a refreshed patient panel will be posted to this site, reflecting updates and changes in your panel. To view your Patient Panel Report, log in to

Care Planning Registry
Available monthly, this registry will provide a list of your patients needing services centered on quality metrics, based on a due date no later than Dec. 31, 2011. This report will be refreshed on or around the 20th of each month. To view your Care Planning Registry, log in to

Baseline Quality Performance Report
Based on your marketing specialty, this report shows your performance in the pay-for-quality program's 10 adult or six pediatric clinical metrics for calendar year 2010.
