    This is the complete guide to HMSA's new and advanced primary care patient-centered medical home (PCMH) program.   • 6.1 Practice Readiness Assessment
    The new PCMH requirement 6.1 Practice Readiness Assessment is now available. We recommend reviewing the questions before starting. The online assessment will take about 30 minutes to complete.

  • 6.2 Transformation Plan
    The new PCMH requirement 6.2 Transformation Plan is now available. The transformation plans must be submitted electronically, using the fillable PDF template. More detailed instructions can be found on the first page of the template.
