Chronic disease can be overwhelming and stressful. For many patients, figuring out how to navigate the health care system adds to the challenge and frustration. HMSA's disease management services are designed to reinforce the care you provide by educating patients and helping them take a more active and responsible role in controlling their chronic disease.
Disease management services are available for HMSA members with asthma, chronic obstructive pulmonary disease (COPD), coronary artery disease (CAD), heart failure, diabetes, or chronic kidney disease (CKD). Services are also available for members with behavioral health conditions.
Eligible members are identified via claims information and automatically enrolled for the services if they have a chronic disease. Members with any of these conditions may also enroll by calling HMSA's Disease Management Services. Members may opt out of services at any time by calling the same number.
In general, all participating members will receive the following services:

Many physicians will be asked to participate if they have patients with any of the aforementioned conditions. Health care providers who choose to participate are offered tools and services that include various newsletters, reports, evidence-based clinical practice guidelines, disease management resources, and other information as provided by the disease management provider service managers to help them manage their patients' conditions. Disease management services are not designed to replace the care members receive from their physician.
