Payment Transformation is HMSA’s effort to change provider compensation to ensure the sustainability of independent providers in a free-choice system. This replaces Pay-for-Quality and Patient-centered Medical Home (PCMH) programs.
Pay-for-Quality is a quality investment program that provides financial incentives for physicians and hospital practices that support the principles of advanced care and achieve targeted goals for improved care delivery.
Patient-centered Medical Home (PCMH) has helped advance provider practices on the journey toward HMSA’s vision of creating a sustainable health care system for Hawaii.
Disease Management Programs promote optimal health for patients through support of patient-physician interaction and ongoing patient education on diabetes, asthma, chronic kidney disease, COPD, and cardiovascular disease.
