The Patient-Centered Medical Home Guide (effective January 2017) provides information for physicians and physician organizations on how to start a PCMH, program objectives, guidelines and requirements, success criteria, and patient materials for reference.
The FQHC Patient-Centered Medical Home Guide (effective January 2017) is now available. Physicians and physician organizations affiliated with a federally qualified health center (FQHC) are eligible for participation in this program.

HMSA also offers an online tool called the Care Planning Registry, through Cozeva.com. This registry provides a list of your patients who need services that are measured as part of the quality metrics.
