Advanced Care Programs support the delivery of quality care and efficient management. These innovative programs help primary care physicians, hospitals, and specialists use best practices, share information, and assess results.

Quality Investment programs provide financial incentives for physicians and hospital practices that support the principles of Advanced Care and achieve targeted goals for improved care delivery. These improvements include quality outcomes, patient safety, and efficiency.

Quality Programs

Due to recent NCQA changes that affect measures and specifications, additional changes to the program guides are needed. Revised guides will be posted by July 24, 2014.

Patient Panel Report
This report is the total list of your patients for the current month, with name and contact information. Each month, a refreshed patient panel will be posted to this site, reflecting updates and changes in your panel. To view your Patient Panel Report, log in to Cozeva.com.

Care Planning Registry
Available monthly, this registry will provide a list of your patients needing services centered on quality metrics, based on a due date no later than Dec. 31, 2011. This report will be refreshed on or around the 20th of each month. To view your Care Planning Registry, log in to Cozeva.com.

Baseline Quality Performance Report
Based on your marketing specialty, this report shows your performance in the pay-for-quality program's 10 adult or six pediatric clinical metrics for calendar year 2010. Going forward, your ongoing performance will be measured and scored against this baseline period. To view your Baseline Quality Performance Report, log in to Cozeva.com.
